How to Access Your W-2


Current employees

Can access W-2 forms electronically through CIS.

To View Your W-2:

  1. Login to CIS (Campus Information Services).
  2. In the search box, enter “W-2”.
  3. Click “Electronic W-2 Forms” to access your available forms.

Former employees

You will receive a paper W-2 by mail.

Access to electronic W-2s is not available, regardless of any prior consent for electronic delivery. For reprints, submit a W2 Reprint Request.

How to Enroll in Electronic W-2 Delivery
  1. Login to CIS (Campus Information Services)
  2. In the search box, enter “Payroll, Taxes, and Salary”
  3. Click the dropdown on “W-2/W-2c”, then select “W-2/W-2c Consent”
  4. Select and submit the option to receive your W-2 electronically.

How to Read Your W-2


Your W-2 form summarizes your taxable wages and the taxes withheld from your university paychecks in a calendar year. It is used to file your federal and state tax returns. Below is an overview of what each box on the form represents

  • The social security number must match the employee’s social security card. If there is a discrepancy, please contact your department HR Representative for record correction and email TaxServices@utah.edu for a corrected W-2.

  • The University of Utah’s federal tax identification number. 

  • The University of Utah’s name, address, and ZIP code. 

  • For employer use only

  • The employee’s name, address and zip code.  

    Employee’s name must match the employee’s Social Security card. If there is a discrepancy, please contact your department HR Representative for record correction and email TaxServices@utah.edu for a corrected W-2. 

    A corrected W-2 is not required for an address change. If your name appears shortened on your W-2, it is due to character limits in the IRS form format. The full name is still associated with your Social Security Number for reporting purposes. 


  • Shows your total taxable income from the university for the 2024 calendar year (Dec 16, 2023 – Dec 15, 2024). 

    It includes: 

    • Wages after pre-tax deductions (gross pay minus amounts not subject to tax, such as retirement contributions, flexible spending accounts, parking, and salary deferrals like 403(b) or 457 plans) 
    • Taxable employer provided benefits (health insurance and education assistance) 
    • Moving Allowance 

    It does not include: 

    • 403(b) or 457 retirement employee contributions 
    • Health or dependent care employee contributions 
    • Parking permit payments 

    This is the amount reported as taxable wages on your individual tax return. 

  • Shows all federal taxes withheld from university paychecks in the calendar year. 

  • Shows total wages subject to Social Security tax.  

    • Includes Box 1 wages plus 403(b) and 457 salary deferrals 
    • Capped at the maximum taxable limit of $168,600 for calendar year 2024 
  • Shows the total amount of Social Security tax withheld from your university paychecks. This appears as FED OASDI/EE on your paycheck. 

    • Calculated as 6.2% of the amount in Box 3 
    • The maximum withheld is $10,453.20 (6.2% of $168,600) for calendar year 2024. 
  • Shows total wages subject to Medicare tax. Unlike Social Security, there is no income cap for Medicare tax. 

    • Includes Box 1 wages plus 403(b) and 457 salary deferrals 
  • Shows the total Medicare tax withheld from your university paychecks. This appears as FED MED/EE on your paycheck.

    • Calculated as 1.45% of Box 5 wages
    • If your wages exceed $200,000, an additional 0.9% is withheld on the amount over that threshold
  • not applicable to the university

  • Shows the amount withheld from your pay and contributed pre-tax to a dependent care flexible spending account.

  • Not applicable to the university

  • Code E: Elective deferrals under a 403(b) salary reduction agreement

    Code G: Elective deferrals to a 457(b) deferred compensation plan

    Code BB: Designated Roth contributions under a section 403(b) plan

    Code DD: Cost of employer-sponsored health coverage. This amount is not taxable.

  • The retirement plan box is checked if the employee is eligible for any university paid retirement plan or the employee actually participated in a salary reduction arrangement under 403(b) or 457.

  • This box includes items for information purposes.

    Amounts deducted before tax (not included in Box 1):

    • Health/FSA – The amount deducted from your pay before taxes for health care and dental insurance premiums. The FSA portion applies to employees who enrolled and deferred wages to a health flexible spending account.
    • PARK – Amounts deducted from your pay before taxes to pay for university parking permits

    Amounts added to taxable wages (included in Box 1):

    • EXLI – Amounts paid by the university for executive life insurance
    • CCAR – Amounts paid by the university for an automobile allowance
    • UME – Amounts paid by the university for a moving allowance
    • TUI – The amount of tuition reduction benefit you received for yourself, a spouse or child that is taxable to you. Tuition reduction benefits you received that are not taxable are not included on your W-2.
    • NRA – Amounts paid by the university for legal fees on your behalf
    • GIFT – Amounts you received from the university as gifts or awards

    COVID-19 related sick leave and emergency family leave wages (Amounts you received from the FFCRA due to COVID-19, subject to IRS limits)

    • Sick leave wages subject to the $511 per day limit
    • Sick leave wages subject to the $200 per day limit
    • Emergency family leave wages
  • This is the state and the University of Utah’s state identification number.

  • Shows all state taxable income from the university in the 2024 calendar year (Dec 16, 2023 – Dec 15, 2024).

  • Shows all state taxes withheld from university paychecks in the calendar year.

  • The state of Utah does not have local wages, tips, etc. If a state other than Utah, amount will be reported if applicable.

  • The state of Utah does not have local income taxes. If a state other than Utah, amount will be reported if applicable.

  • The state of Utah does not have locality taxes. If a state other than Utah, amount will be reported if applicable.

W-2 Frequently Asked Questions (FAQ)


  • Current Employees – Your electronic W-2 will be available by logging into CIS. In the Search box, enter “W-2” and click the “Electronic W-2 Forms” link. If you did not consent to receive your W-2 electronically, your W-2 will be mailed to the current home address in CIS by January 31.

    Former Employees – A paper W-2 will be sent to all former employees. All former employees, regardless of electronic W-2 consent, will no longer be able to access the electronic W-2.

    All printed W-2 forms are postmarked by January 31. Please allow two weeks to receive the W-2 via the U.S. Postal Service. If you did not receive or lost your printed W-2, please refer to the W-2 Reprint Request or email TaxServices@utah.edu.

  • Your name and SSN must match the information on your Social Security card. Please email TaxServices@utah.edu for instructions to request a W-2 correction form.

  • A corrected W-2 is not required for a change in address.

    If you need to change your address:

    • Sign on to the CIS (Campus Information Services).
    • In the upper-right corner menu with your name, click on the “Employee Profile” or “Affiliate Profile” link.
    • Click on the Home Address section.
    • Make changes on the Address page.
    • Click on “Save” in the upper right-hand corner to save the changes.

  • The W-2 reflects taxable earnings while the check stubs reflect total earnings. To convert from total earnings to taxable earnings, you need to subtract the nontaxable deductions. These include parking deductions, FSA deductions (medical and dental deductions, flexible spending account deductions and dependent care deductions) and retirement deductions (403(b), 457). These deductions are itemized in Boxes 12, 13 and 14 of the W-2.

    Your Form W-2 is a summary of the taxable earnings received in a calendar year. For the calendar year 2024, paychecks for pay period ending December 31, 2023 through December 15, 2024 are included on the 2024 W-2.

  • Yes, this is correct. Form W-2 is issued on a cash basis. Any compensation paid to you in 2024 is considered 2024 income. The final half of December was paid on January 5, 2025 and is therefore not considered income for 2024. It will be part of your 2025 Form W-2.

  • Your W-2 does not reflect your fiscal year salary; it reflects taxable, calendar year wages. The fiscal dates that your W-2 covers are from December 16, 2023 to June 30, 2024 (13 pay periods) and July 1, 2024 to December 15, 2024 (11 pay periods).

  • Please see the description of what taxable wages are reported on Form W-2, Box 1, Wages. If you still think there is a problem, please contact your department’s Payroll Reporter to determine if you were paid correctly for the calendar year.

  • FSA stands for Flexible Spending Account and includes items like medical and dental deductions.

  • When you elect your medical and dental plans you are automatically enrolled in FSA. This provides that your health insurance premium payments are on a pre-tax basis. The pre-tax payroll deduction can be coded as “FSA Dependent”, “FSA Health” or “FDA Medical” on your paycheck. If you see “Health/FSA” in Box 14 but do not have any FSA deductions in the calendar year, the FSA part does not apply to you.

  • Refer to the 'Box 14' subsection within the 'How to Read Your W-2' section above.

  • You may not have had any federal and state withholding taxes if you claimed a withholding exemption, dependents and/or made adjustments for other tax credits/deductions on your W-4 form for the year (Employee’s Withholding Certificate). Check out your tax status through CIS W-4 Tax Information. If your earnings are under a certain threshold, you may not have been subject to any federal and state withholding taxes. You can change your CIS W-4 to adjust your federal withholdings.

  • Nonresident aliens who qualified for a treaty exemption and completed the necessary paperwork may receive a Form 1042-S and/or Form W-2 showing their treaty exempt earnings/other wages and taxes. Form 1042-S is required before you can complete your income tax return. Forms 1042-S are required to be delivered by March 15.   • No. You will receive one W-2 that includes the combined wages and taxes withheld for all university jobs including the University of Utah Hospital during the calendar year.

  • A W-2c, or Corrected Wage and Tax Statement, is issued when there is a correction to your original W-2 form, such as adjustments to your wages, tax withholdings or other reported information. If you are an active employee, your W-2c can be viewed online through CIS. If you are no longer active, a paper W-2c will be mailed to the address on file. The W-2c should be included in your tax return filing and it may impact your taxes if the corrections are significant. It is important to review the form carefully and consult a tax advisor if you have any questions.

Payroll Deductions Before Taxes


 

The deductions listed below are taken from your pay before federal and state taxes are calculated. This means you don’t pay taxes on these amounts.

To reconcile your taxable wages with your gross year-to-date salary (as shown on your December 22 paycheck), add these pay codes back to your taxable wages.

Note: The calendar year for tax purposes ends on the pay period that ended December 15. Wages are reported based on the pay date, not when they were earned. Pay for the period ending December 31 is issued on January 7 and counts toward the next calendar year’s taxes.
